To
act as mentor, support, teacher, advisor and assistant to the person
they work for. They are the most vital member of the support team and
are in the best position to identify what supports are needed for a
person to succeed and to provide those supports.
•
To
Coordinate scheduled times to be worked with the person with input from
the program manager. Become familiar with the person’s support strategy
plan including behavior supports. Actively implement and document the
supports to ensure the person’s needs and goals are being met..
•
To
Document progress, successes and any incidents daily. Complete monthly
reports (with the person), and program notes to ensure they maintain a
high level of quality. Assist program managers with written summaries
to support coordinators. Keep essential medical emergency and personal
information (such as changes in address, phone, parent phone, primary
physician, etc.) current and available. Complete Incident Reports
accurately and ensure they are complete and submitted in a timely
manner.

To Be an advocate for the person’s rights and comply with our
“Confidentiality Policy #II-F.”
•
To Connect people with the community and see that opportunities
are provided for developing relationships/social skills.
•
To be the communication hub for the person you serve and for the
team supporting that person.
•
To
Return pages promptly. (Check pager and voicemail at the beginning and
end of each day. Return urgent messages as soon as possible.
•
To Record time worked as per our “Reporting of Hours Policy
#III-G,” and ensure it is done at the required times.
•
To
Schedule medical appointments, and educate the person you serve on
important issues. Know the difference between urgent and regular
medical care. Know the medications and side effects for the person you
support. Understand the impact of medical problems such as seizures,
diabetes, etc. Take health care visit forms to appointments, assure
that completed documentation is returned to the nurse consultant and
communicated to the program manager.

To Be aware of and take responsibility for the individual’s
health and safety (shelter, food, health).
•
To
Study the “Financial and Shopping Policy #II-J.” Ensure the accurate
maintenance of financial records for the person they support. Work with
the eligibility specialist to assure the policy is met. Keep property
inventories current and available.
•
To
The primary staff is accountable for keeping all aspects of the
individual’s finances in compliance with the “Financial and Shopping
Policy #II-J.” They must submit all key paperwork appropriately. The
primary staff will meet the paperwork requirements to protect Social
Security, Medicaid, housing, and other benefits for the person in
services. The primary staff should take the initiative to obtain
training on these issues from the eligibility specialist and the
program manager..
•
To
Become familiar and take responsibility for knowing the following,
through the use of the company policy manual: “Problem Resolution
Policy #II-K,” teamwork as in our “Team Policy #III-V,” and the
financial, medical and communications policies as listed above..
•
To
Attend all staff meetings and other offered training. Be current on all
CPR/first aid and other basic training requirements. Check with human
resources department to ensure your personnel file is complete and
current.
•
Must be 18 years or older.
•
Must Must be able to work a flexible schedule to meet the needs
of the people we serve.
•
Personal vehicle preferred or reliable transportation.
•
Must Must be able to lift up to 20 lbs. may require more lifting
if the needs of the person are present.
•
Must Must present current MVR before hiring. After hiring, must
present current driver’s license and proof of auto insurance.
•
Positive
pro-active attitude. Strong interpersonal and organizational skills,
including compassion, self-confidence, honesty, integrity,
accountability, with the ability to follow-up and be compatible with
our people.
•
Must have strong work ethic, good writing skills, and patience
to fill out detailed therapeutic documentation.
•
Must complete a health screening and obtain a TB test upon hire.
•
Must obtain CPR and First Aid certification within first three
(3) months of employment.
