Resource Facilitation is a free, two-year support program that provides education and connection to supports and services to assist people throughout Utah in navigating life after brain injury.
After an initial meeting with the individual and their family, the resource facilitator will assist with:
help problem-solve issues and identify resources to help them transition back to family life, work, school, and the community while achieving the greatest level of independence as possible.
Collect medical documentation to qualify for identified resources.
Develop a plan to create goals and establish needed supports.
Weekly contact to ensure that goals are being addressed and accomplished.
